Appointment slots are limited and will take place at Edna Martin Christian Legacy Center located at 2259 Ralston Ave, Indianapolis, IN 46218.

Registrants should be over 40 unless doctor referred. A phone call to share pre-appointment information is required to confirm your appointment time. Please complete your pre-appointment phone call within 3 business days of signing up or your space could be relesed to another registrant.
